The webinar will include information on the bi-directional relationship between depression and diabetes, including the psycho-social relationship and some of the physiological contributions. The presenters will also discuss ethnic disparities, best practices and recommendations for addressing these issues, including management in a telehealth environment. 

About the Presenters:

Dr. Andy Wong practices Adult Psychiatry at Kaiser Permanente, Baldwin Park, and is currently piloting a program integrating psychiatry with family medicine. Additionally, he serves as the psychiatry champion for the Kaiser Permanente Depression Care Management team in the Baldwin Park location. He conducted his Psychiatry residency at the Los Angeles County University of Southern California Medical Center and previously practiced as a general practitioner in primary and urgent care.

Dr. Paul Magtoto practices family medicine at Kaiser Permanente in the Montebello location. He serves as the Complete Care Physician Lead for the San Gabriel Valley. He places a strong importance on integrating dignity and compassion in his medical care, and has served his Kaiser patients since 2000. He obtained his medical degree from the University of California, Irvine, and completed his internship and residency at Glendale Adventist Medical Center.

Health Integration Summit Part II-May 7, 2019

The Consortium held its second Health Integration Summit on May 7, 2019 1:00-4:00pm at the City of Hope.  The day’s objectives were for providers to forge new collaborations for networking to further enhance integration of physical health, mental health and substance use services in the Greater San Gabriel Valley; learn about existing models for integrating services; identify actions they can take to better integrate services; and learn about new ways to access community resources.

Target audience were representatives from service organizations who have an understanding of program implementation, operations and/or clinical services, and who can make decisions and share ideas and suggestions with their organizational leadership.  Providers were able to display organizational materials, and network with other providers.  

Integration Summit II was kicked off with introductions and opening remarks to define integration and clarify objectives for the day. Participants hoped the Summit would help them establish best practices for integration, increasing knowledge about services and resources, and increasing collaboration and partnerships.   They reported their organizations are already engaged in networking, cross-staff training, case management, patient navigation, and warm hand-offs.  Key integration gaps they listed include:   Lack of communication/coordination, funding/capacity, language/cultural barriers, stigma, data sharing, and training needs. 

The day’s presentations covered the following topics:

  • Social Determinants, Sectors & Systems: Creating a Healthy SGV
  • Behavioral  Health Integration in Primary Care, LA County
  • Expanding Access to Treatment for Opioid Use Disorders
  • One Degree resources

Attendees participated in breakout sessions to discuss building health integration, and identified  themes and priority areas for attention such as:  Education and cross-training, formalizing relationships (MOUs), capacity building, collaboration and partnerships, resource building, data exchange, improving referral processes, sustainability and funding, awareness and advocacy at the state level, provider champions (across service sectors), identify missing population and bring them to the table (i.e. Faith-based organizations; Seniors/Dementia programs; Law enforcement), and addressing needs for high-vulnerability populations (e.g. SUD, LGBTQ, homeless).

Health Integration Summit, Part I-March 5, 2019

The first of two Health Integration Summits were held on March 5, 2019 in Irwindale, CA.

The overall goal of the Health Integration Summits is to forge new collaborations, partnerships and opportunities for networking that will further enhance integration of physical health, mental health and substance use services in the Greater San Gabriel Valley.

The Health Integration Summits are targeted to representatives from physical health, mental health and substance use service organizations who are in leadership positions and who have an understanding of program implementation, operations and/or clinical services.Due to limited capacity, we request that each organization limit registrations to no more than three people.

Agenda highlights for Summit I:

  • Panel presentation on Integration Issues & Challenges in SPA 3 from the L.A. County Perspective, including presentations from Michelle Gibson (Acting Deputy Division Director for Treatment Services, LADPH/Substance Abuse Prevention & Control), Lisa Wong, Psy.D. (Mental Health Clinical Chief, Service Area 3, Department of Mental Health), and J. Michael Allevato, M.D. (Medical Director, SGV Health Center Group, Department of Health Services).
  • Panel presentation on Health Neighborhoods as a Best Practice Model of Integration in SPA 3, including prsentations from DMH, Social Model Recovery Systems, Inc., ENKI Health & Research Systems, Inc., Mission City Community Network and Our Saviour Center.
  • Opportunity for networking and strategizing in small groups on how to best serve clients/patients with multiple, cross-system needs and how issues of referral relationships, co-management/coordination of care and data sharing impact integrated care.

Resource Materials – All participants are welcome to bring 130 copies of resource materials to share, limited to 3 pieces of information per organization. This will be an informal process. Tables will be available for resource materials and organziations are welcome but not required to have a staff person available during the registration period and the break to answer questions.
