Our mission is to strengthen the healthcare safety net and optimize seamless access to high quality physical and behavioral health in the Greater San Gabriel Valley.

History of the Consortium:

  • In 2000, a grant from the Department of Public Health sparked collaboration in the Greater SGV​. Providers coalesced as the SPA 3 Health Planning Group and the group decided to keep working together!​
  • Steering Committee has met monthly or bi-monthly ever since.
  • 2005-2010 initiated and implemented a collaborative Retinal Telemedicine program to screen for diabetic retinopathy among clinic patients from across the SPA.
  • In 2010 received first multi-year grant – Specialty Care Initiative funding from Kaiser Permanente, including a CRC prevention program with clinic partners.
  • ​In 2014 entered into fiscal sponsorship agreement with Public Health Foundation Enterprises, Inc. (dba Heluna Health) to provide contracting & financial administration services.
  • Since then, received funding from CCF, UniHealth Foundation, Kaiser Permanente, Health Net, Ralph M. Parsons and others to engage in organizational development planning and provide numerous programs to support our community.

  • Build the presence and visibility of the Health Consortium in the wider community through strategic partnerships and improved communications, both internally and externally
  • Invest in our Infrastructure & a Financial Model
  • Invest time and resources in development of infrastructure and a financial model to ensure long-term sustainability
