Established in 2000, Health Consortium of Greater San Gabriel Valley is dedicated to serving vulnerable populations, supporting access to services, information sharing, promoting strategic partnerships and increasing visibility for our participants. Our mission is to strengthen the health care safety net and optimize seamless access to high-quality care for physical health, mental health, and substance use disorder services in the Greater San Gabriel Valley.

The Health Consortium’s service area aligns with L.A. County’s Service Plan Area 3 (SPA 3) which stretches along the foothill communities from Pasadena to Pomona and includes the areas both north and immediately south of the 10 Freeway.
