Hospital Collaborative Committee

The Health Consortium convenes and facilitates the Greater SGV
Hospital Collaborative, a group of six area nonprofit hospitals and two
public agencies. The Hospital Collaborative partners are committed to
developing a coordinated set of regional strategies to address shared
priorities that have emerged from their Community Health Needs
Assessments (CHNAs), and also to coordinating and sharing data
across their respective CHNAs. Mutual priority issues across the San
Gabriel Valley (SGV) region include food insecurity, homelessness,
recuperative care and access to mental health services. A Snapshot
report highlighting high-level data about the area’s population and five
priority issue areas from the 2019 CHNAs was presented at the Current
Health Trends in the Greater SGV event held at City of Hope in
February 2020. This report and links to each of the hospital’s most
recent CHNAs can be accessed via this link on the LADPH website.


As well, the Hospital Collaborative led the development of the Food4All
SGV program, funded by UniHealth Foundation for three years through
2024, to link hospital patients screened as food insecure to patient
navigators at two food organizations, Seeds of Hope and Project Angel
Food, for linkages to food, food benefit programs, and other services.
The Hospital Collaborative also was a catalyst to participate in a pilot
for the SPA 3 Patient Navigation program in partnership with United
Way of Greater LA and Union Station Homeless Services. This program
connects high-user, unhoused patients with patient navigators upon
hospital discharge to facilitate connections and referrals to housing,
health care, and other immediate needs.


Greater SGV Hospital Collaborative partners include City of Hope,
Emanate Health, Huntington Hospital, Kaiser Permanente Baldwin Park,
USC Arcadia Hospital, Pomona Valley Hospital Medical Center, Los
Angeles County Department of Public Health (LADPH) and
Pasadena Public Health Department. Technical support is provided by
Center for Nonprofit Management
