Food for All SGV

The Health Consortium’s Food for All San Gabriel Valley (Food4All SGV)
program is a collaborative program to help address food insecurity among
patients served at five of the six local hospitals participating in the Hospital
Collaborative. The goal of this program is to link hospital patients screened
as food insecure at the five partner hospitals with emergency food resources
and medically tailored meals (MTMs). Currently in its first year of a three-
year funding commitment from UniHealth Foundation, the primary objectives
are to 1) build program infrastructure and capacity, such as developing
hospital teams and patient screening/referral workflows, evaluation
planning, planning with two referral platforms (Unite Us and Findhelp) for
integration at the hospital and food partner sites, working with the food
partner organizations to clarify the Food Security Navigator roles and the
food resources that each can offer; 2) provide emergency food to 1,200 food
insecure people in partnership with Seeds of Hope and 12,500 medically
tailored meals in partnership with Project Angel Food; and 3) plan for
program sustainability.

Full Group Meetings:

Our bi-monthly Full Group Meetings are every other month, on Tuesday, from 1-3pm. All are welcome to join, and we ensure our presentation topics are applicable to health providers in the San Gabriel Valley area. Our 2021 and 2022 topics included:

Health Integration Webinar Series

Funded by the UniHealth Foundation, we implemented our Health Integration Webinar Series, which included seven webinars. These events provided educational presentations, resources, data, and policy updates in response to identified priorities at Full Group meetings and/or other convenings.

SPA3 Patient Navigation for Unhoused Frequent Hospital Users Pilot

In 2019, due to the reputation of the Health Consortium and the SPA 3
Hospital Collaborative (led by the Health Consortium), United Way of
Greater Los Angeles approached the Health Consortium to initiate
planning for a SPA 3 Patient Navigation pilot program in partnership with
Union Station Homeless Services. Fully operational since the beginning of
2021, the program provides post-discharge housing and health care
linkages and support to unhoused individuals who are frequent users of
five area hospitals. Of the 287 referrals that have been made by the
hospitals to the Patient Navigators, 95 patients have been enrolled and
received services, including linkages to crisis beds for temporary and
permanent housing as well as to health care services.
