Food4All SGV Program (2021-2024)
- Engaged five hospitals and two food organizations to build relationships and collaborate on a successful, regional program to address food insecurity.
- Hospitals referred 460 patients to Seeds of Hope and 381 patients to Project Angel Food.
- Project Angel Food provided over 12,500 medically tailored meals to 180 program patients.
- Seeds of Hope provided 200 referrals to food programs (e.g., food benefits, meal delivery services, food banks) and 75 referrals for other patient needs such as rental assistance, utility assistance and transportation.
SPA 3 Patient Navigation for Unhoused Frequent Hospital Users Pilot (2021-2023)
- Program partners reported there was teamwork and collaboration across organizations, stronger advocacy for patients and better use of resources.
- Union Station Patient Navigators served 180 patients from five hospitals, including identifying crisis beds for temporary and permanent housing, linkages to primary care providers, access to mental health substance use disorder services, arranging transportation, obtaining documents needed to apply for housing, and other services.
