Mission
To support and strengthen the local network of physical, behavioral, and social health organizations to promote equity and to facilitate integrated, quality, and culturally responsive services among diverse and vulnerable communities in the Greater San Gabriel Valley.
Strategic Goals for 2025-2030
- Build community partner capacity to provide quality, equitable and culturally responsive care, while enhancing service integration across partner organizations.

History of the Health Consortium
2000
A grant from the Department of Public Health sparked collaboration in Service Planning Area 3 (SPA 3, i.e., the Pomona and San Gabriel Valleys). Providers coalesced as the “SPA 3 Health Planning Group.”
2001
The Group conducted its first strategic plan focused on access to primary and specialty health care for underserved communities and established a Steering Committee to guide organizational strategy and decision-making.
2002
Launched bi-monthly “Full Group” meetings, held in-person or virtually, creating an inclusive forum for all Health Consortium partners and participants to support the delivery of high-quality, integrated, and equitable health and behavioral health services to vulnerable populations in the San Gabriel Valley (SGV).
2005-2010
Initiated and implemented the collaborative SPA 3 Retinal Telemedicine program to screen for diabetic retinopathy among clinic patients from across the area. East Valley Community Health Center provided the screenings at no cost to patients from other local community clinics.
2010-2014
Received multi-year Specialty Care Initiative funding from Kaiser Permanente, including a colorectal cancer prevention program with clinic partners.
2014
The organization was renamed to Health Consortium of Greater San Gabriel Valley and entered into a fiscal sponsorship agreement with Heluna Health for contracting and financial administration services.
Mission updated to include a focus on integrating physical and behavioral health care.
2018 – Present
Convened SGV Hospital Collaborative, bringing together six nonprofit hospital partners to coordinate Community Health Needs Assessments (CHNAs) and address shared community health concerns across throughout the region.
2020-2023
The Health Consortium partnered with United Way to launch the SPA 3 Patient Navigation for Unhoused Frequent Hospital Users Pilot. This initiative connected unhoused patients with navigators from Union Station Homeless Services upon hospital discharge, ensuring they received essential housing and healthcare services.
2020-2021
Transitioned to a virtual meeting format and launched the Health Integration Webinar Series, funded by Health Net, to provide trainings, educational presentations, resources, data, and policy updates to support providers during the COVID-19 pandemic.
2021-2024
The Health Consortium established and directed the Food for All San Gabriel Valley program, funded by the UniHealth Foundation. This initiative united five SGV hospital partners and two food organizations to tackle food insecurity among vulnerable hospital patients.
2022
Hosted the first San Gabriel Valley Health Summit: Behavioral Health in the Greater San Gabriel Valley: Trends, Equity and Interdisciplinary Collaboration.
2024
Hosted the 2024 San Gabriel Valley Health Summit at the SCE Energy Education Center in Irwindale, emphasizing Diversity, Equity, and Inclusion to address health and behavioral health priorities in the SGV.
2025
The 2025 San Gabriel Valley Health Summit, Exploring Health Issues, Challenges, and Opportunities in the Aging Community, will be held on April 10, 2025, again at the SCE Energy Education Center in Irwindale.
