San Gabriel Valley Hospital Collaborative

The Greater SGV Hospital Collaborative brings together partners including City of Hope, Emanate Health, Huntington Hospital, Kaiser Permanente Baldwin Park, USC Arcadia Hospital, Pomona Valley Hospital Medical Center, Los Angeles County Department of Public Health (LADPH), and Pasadena Public Health Department.

Convened by the Health Consortium, the SGV Hospital Collaborative is dedicated to coordinating and sharing data across their respective Community Health Needs Assessments (CHNAs) and to developing collective strategies that address regional priorities. Key issues identified include food insecurity, homelessness, recuperative care, and access to mental health services. In 2020 and 2023, high-level data snapshot reports spotlighting these and other Social Determinants of Health were presented at the “Current Health Trends in the Greater SGV” events held at City of Hope. The report and links to the latest CHNAs are accessible on the LADPH website.

The SGV Hospital Collaborative has spearheaded programs and initiatives addressing these shared priorities:

  • Food4All SGV (2021-2024): Funded by UniHealth Foundation, this program focused on connecting hospital patients screened as food insecure to patient navigators from Seeds of Hope and Project Angel Food. Services included linkages to healthy food, food benefit programs, Medically Tailored Meals, and other services.
  • SPA 3 Patient Navigation for Unhoused Frequent Hospital Users Pilot (2021-2023): In partnership with United Way of Greater LA and Union Station Homeless Services, this pilot supported unhoused, frequent hospital users from five area hospitals by providing housing and healthcare linkages post-discharge.

New and Ongoing SGV Hospital Programs

Community Health Needs Assessment Coordination

The tri-annual Community Health Needs Assessments required of all nonprofit hospitals are coordinated to facilitate information sharing and minimize duplication of efforts.

Food4All SGV Program Expansion

With support from the LA Food Equity Fund, the Health Consortium is expanding Medically Tailored Meals (MTMs) through Food4All SGV. In partnership with Project Angel Food, this initiative will now serve Medi-Cal patients eligible for MTMs under CalAIM from both our hospital partners as well as from local Community Health Centers, expanding the provision of critical nutrition support for individuals with chronic diseases in the San Gabriel Valley. A Project Angel Food navigator will work with referring entities and patients to ensure smooth referral and benefits linkage through CalAIM.

Recuperative Care Access in the SGV

The SGV Hospital Collaborative advocated for recuperative care services to support homeless patients who need a safe place to recover post-hospitalization. Horizon Recuperative Care, set to open in El Monte in December 2024, will be the first provider in the San Gabriel Valley.

Address Social Determinants of Health

Starting in 2024, nonprofit hospitals are mandated to assess and address social determinants of health (SDOH) needs among their patients. To support these efforts, the Health Consortium will engage community-based organizations to share information about their services and outline how hospitals can refer patients to access them.
